Has anyone used/created a shortened version of NACCHO's Local Public Health Systems Assessment tool? It seems we are always surveying for this or that, so we would like to be able to capture key areas of the assessment (assessing the 10 ESPHS) with fewer questions, yet find it challenging. Just curious if anyone has used a different, shorter tool to capture this information?
